Parent Permission and Registration Forms

Use this form to enroll your children in swim lessons and athletic programs, and also to participate in Clubhouse activities and events. Register your children for swim lessons using this form. Remember, if your children are members of the Highland Lakes Swim Team, you must also complete the swim team’s registration and permission form. Completed forms may be mailed to or dropped at the Club office (there is a mail slot in the door).
